Special Olympics Return to Activity

As we Return to Play, SOKS wants to try and assist Local Programs with Personal Protective Equipment in order to begin to have safe practices. While we can’t guarantee that we can fulfill all of your requests, we will do our best. Please contact Chris Burt at burtc@ksso.org for assistance with PPE.

Special olympics is requiring some additional forms/training to be completed prior to beginning practices.

Communicable Diseases Waiver – Required for all Athletes, Coaches, Unified Partners and Program Volunteers prior to attending an in-person practice or competition (EFFECTIVE MARCH 1, 2021)

COVID-19 Participant Code of Conduct and Risk Form (PDF) – Required for all Athletes, Coaches, Unified Partners and Program Volunteers prior to attending an in-person practice or competition

COVID-19 Clearance Form – Required for all Athletes that have had a postitive test before they can return to activity

NFHS Course – Covid 19 for coaches and administrators  – We are requiring all Certified Coaches/Assistant Coaches/Program Assistants to complete this training.  Once you completed this training, please return the Certificate to johnsonm@ksso.org.
